Franklin County Public Health

Franklin County Public Health provides a list of healthcare resources for adults with limited incomes. Additionally, their Care Coordination Program connects Franklin County residents to medical and social services throughout the community. Clients are partnered with a Community Health Worker who will help you to access any resources you may need. To be connected to a Community Health Worker, call (614) 525-4892 or complete this form. We are in the process of trying to have a Community Health Worker visiting the New Albany Food Pantry on a regular basis to assist our clients.
